Home Telehealth

Home Telehealth (HT) utilizes case management principles to coordinate disease management protocols by means of health informatics such as in-home and mobile monitoring, messaging and/or VA Video Connect. The HT program will help Veterans learn more about how to manage their condition. The goal of HT is to improve clinical outcomes and access to care while reducing complications, hospitalizations, and clinic or emergency room visits in post-acute care settings, high-risk Veterans with chronic disease or at risk for institutional long-term care, and those who will benefit from additional health promotion and prevention activities. As of April 2019, there are 73,908 patients enrolled in this program nationally.

The HT program, in alignment with the Mission Act, assists the clinician to monitor the Veterans’ health from the home. The daily ongoing care assessment and health informatics technologies enhances and extends current VA care management activities into non-VA settings and the Veterans’ residence.

Enable shared decision-making between patient and practitioner across the continuum of care. Support patient-self management and an individual's responsibility for their health. Provide an evidence-based platform to promote the health of the Veteran patient community.
