VA New Jersey Health Care System
Virtual Health/Telehealth
Clinical Video Telehealth (CVT) is defined as the use of technologies to provide clinical care, patient education, provider education and hospital administration in circumstances where distance separates those receiving services and those providing services. A licensed independent health care provider directs, diagnoses, or otherwise provides clinical treatment. Video to Home is a form of CVT in which secure CVT technology is used to allow the veteran to receive treatment in the home
Asynchronous Telehealth is a process by which clinical information (e.g. data, image, sound, video) is acquired and stored, then forwarded to (or retrieved by) another site for clinical evaluation. The ability to acquire, store and then forward digital images for reporting is a crucial area of care coordination development within VHA. Please visit VHA Telehealth Services for more information about CVT or SFT.
VA Video Connect is a VA solution that enables Veterans to virtually meet with their VA healthcare providers, in something called a virtual medical room, using encrypted video to ensure the session is secure and private. It allows Veterans to see and talk to their health care team from anywhere, making VA healthcare more convenient and reducing travel times for Veterans, especially for those in very rural areas with limited access to VA health care facilities.
Home Telehealth (HT) applies care and case management principles to coordinate care using health informatics, disease management and Home Telehealth technologies to facilitate access to care and to improve the health of Veterans, with the specific intent of providing the right care in the right place at the right time.
The goal of Home Telehealth is to improve clinical outcomes and access to care while reducing complications, hospitalizations, and clinic or emergency room visits for Veterans in post-acute care settings and high-risk patients with chronic disease.
My HealtheVet/Secure Messaging is a secure, web-based portal that allows Veterans to easily access their personal health record as well as provide information, tools, and resources to increase Veterans’ involvement in their health care. Web technology combines essential health record information enhanced by online health resources to enable and encourage patient/clinician collaboration that optimizes Veterans' participation.
